Transitional care provides post-acute skilled nursing care and rehabilitative services to help individuals return home or to an alternative level of care successfully. Our focus on transitional care is to maximize recovery by tailoring a plan of care that meets the needs and goals of each individual.
Residents can receive ongoing IV therapy, wound care, physical therapy, occupational therapy, speech therapy, and respiratory therapy. Our therapy services are provided by skilled therapists who focus on functional resident goals and use a variety of equipment and modalities as part of their daily treatments.
