Valley Transitions is a rehabilitation community within Valley Senior Living on Columbia. When we say we provide transitional care, we mean our goal is to get individuals back home or to an alternative level of care. At Valley Transitions, our focus is on maximizing recovery, so we tailor our care plans to meet the unique needs and goals of each individual.
Valley Transitions has 30 private and three semi-private rooms. Every room is spacious, featuring a private bathroom with an accessible shower, a flat-screen television, a telephone, and Wi-Fi internet access.
Our care partners work closely with residents’ personal physicians to design a customized care plan that ensures the most complete recovery possible.
Transitional care is for individuals recovering from illness, surgery, or life-altering events who require specialized care before they’re able to return home or transition to a higher level of independent care.
